In a medium bowl, pour water over gelatin and sugar stir until dissolved.
In a large mixer bowl at medium speed, beat cream cheese until fluffy. At low speed, slowly add gelatin mixture and beat until smooth. Stir in pineapple, apples, celery and nuts. Pour into a 13 x 9 x 2-inch baking pan. Cover and chill at least 4 hours or overnight.
Cut and serve on lettuce if desired.
