Place mayonnaise, sugar, vinegar, horseradish, and salt in a large bowl. Whisk until blended. Stir in cabbage, celery, apple, and raisins until well coated. Cover and chill at least 1 hour or up to 6 hours.
Just before serving, stir in walnuts. If desired, garnish with apple slices.
